A city this empty does not have many people left in it. The ones who are still moving matter more for it.

Small, fast, and outnumbered.
The rat
A young sewer rat that survived the outbreak by being underground when it happened. Agile, curious, intelligent and resourceful — with no advantage over anything hunting it except size, speed and a very good nose. Its name has not been settled yet.
What is waiting
The things in the way.
The Alpha Infected
The first creature the virus ever touched, and the reason it spread the way it did. It has not left the laboratory. It is between you and the antidote.
The Nest Mother
What the sewers built while the city was busy dying. The first thing down there that cannot simply be avoided.
Who is left
Not quite alone.
The Elder Rat
Knows the tunnels, remembers what the city was like before, and teaches you what the game will not stop to explain.
Scientist recordings
A voice left behind on the facility’s systems. The closest thing to an account of what actually happened, delivered by somebody who did not get out.
Survivors and drones
Animals that have not turned, and laboratory drones still following instructions nobody is left to countermand.
Abilities
Everything the rat can actually do.
Sniff
Reads what passed through a space before you did — where an enemy went, where food is, which pipe is worth crawling into.
Climb
Pipework, wire, brick. Most of the route up is climbable if you can find the first hold.
Crawl
Into gaps nothing chasing you can follow through.
Swim
Flooded chambers are routes, not walls — though not everything in the water is dead.
Sprint
Fast, and expensive. Stamina is a real resource and running out of it in the open is how runs end.
Double jump
Unlocked later, and it reopens the map behind you — earlier zones hold routes you could not reach the first time.
Attack
A last resort. You are small and there are more of them.
Craft
Turns scavenged junk into the things that hold the infection back.
What you manage
Three meters, always running.
Health
Spent by contact with the infected and by hazards. Restored by food you have to go looking for.
Stamina
Spent by sprinting, climbing and swimming. Refills when you stop — which is not always an option.
Infection
Rises near the infected and does not fall on its own. Managing it is what crafting is for.
Experience
Earned by surviving, exploring and solving. XP is half of what opens the next zone — the other half is an object you have to go and find.
